Case Management Approach

The hallmark of all CO-OP programs and services is our highly effective Case-Management Approach.
All of our staff and volunteers are trained to provide individualized case-management services
to each client who enters our office. If we do not have a program that meets your need, we will provide
you with a referral to a local agency that can assist you.

Service Delivery Model

Each of our programs have been designed around the CO-OP Service Delivery M odel. This model has
been identified as a way to streamline service delivery create continuity for clients participating in multiple
programs.

Family Atmosphere

Over the years, CO-OP become like a family to many of our clients. We create this Family A tmosphere
by working together as team to make our experiences memorable for everyone. It takes a great deal of
trust, longevity, and mutual respect to operate our programs. We are a people driven agency with an
office that feels more like a home.

Outreach Initiatives

We provide a variety of O utreach I n itiatives to the community. These activities range from resource
referrals and partnership opportunities to our annual open house and CPR Trainings.
